Specialist Equipment The forms in this section are available to eligible child care services (excluding in-home care) to assist the inclusion of a child that has demonstrated an ongoing need for a high level of support in a child care environment. It includes the Application Form, Loans Procedure, Policy and Procedures, Sample SSP and Sample Loan Agreement Form. Inclusion Support Agencies The forms in this section are available to Australian Government approved Child Care Services, including both private and community centre based long day care, outside school hours care including vacation care, family day care, occasional care, and in-home care. It is also available to Australian Government funded non-mainstream child care services (non-Child Care Benefit) such as flexible/multifunctional services, Multifunctional Aboriginal Children's Services (MACS), non-mainstream outside school hours care, non-formula funded occasional care (100 per cent Australian Government funded) and mobile child care services. The forms included here are Inclusion Support Application Form, Acknowledgement of funding, Final Review Form and Variation Application. Membership Application Forms These Application Forms are for those eligible for Group Membership, Country Membership, Special Needs, Community Membership, ABIQ (Autism Behaviour Intervention Qld) Membership and PSCQ Membership. |
